The forms below should be used to provide the school with accurate information and consent.

Child Leaving Form
If your child is leaving Cannon Lane Primary School, please complete this form and return to the school office in advance of the child’s last day at Cannon Lane

Parent Consent to Medication Form
The ‘Parent Consent to Medication’ form is required to be completed and signed by a parent if a child requires medicine or drugs to be administered by authorised school staff during school hours.

Absence Request/Leave of Absence Form
If a child will be required to be absent from school for any reason, parents or guardians must complete the ‘Absence Request/Leave of Absence’ form and hand it to the school office in advance of the absence date.

If your contact details change or need updating, please complete the Contact Information Form and hand it in to the school office.
